19-39 Years Old

SCREENINGS AND TESTS COVERED:

  • Depression Screening (one per year)
  • Height and Weight (BMI)

IMMUNIZATIONS COVERED:

  • Tetanus-diphtheria booster every 10 years
  • Measles, mumps, rubella (MMR): for persons born after 1956 who lack evidence of immunity to measles
  • Varicella (chickenpox) – 2 doses 4-8 weeks apart: (for persons with no history of chickenpox)
  • Hepatitis B – 3 doses for those 24 or older: who have not been immunized previously HPV (males and females to age 26 years old)

TYPE I OR TYPE II DIABETICS:

  • HbA1c Level
  • Diabetic eye exam by eye care professional

HIGH-RISK IMMUNIZATIONS FOR PERSONS WHO MEET HIGH-RISK CRITERIA FOR:

  • Influenza FLU mist (between age 2 to 49)
  • Hepatitis B (age 25 or older)
  • Pneumovax
  • Meningitis (meningococcal vaccine)
  • Hepatitis A
  • LYMErix (Lyme Disease)

OVERALL PHYSICAL AND MENTAL STATUS ASSESSMENT, INCLUDING:

  • Clinical breast exam
  • Clinical testicular exam
  • Complete skin exam
  • Serum cholesterol/HDL every year
  • Urinalysis screening
  • Pap Smear
  • PSA and DRE (age 40 and over)
  • Hemoglobin and hematocrit (CBC)
  • HIV testing
  • Mammogram (age 35 and over)
  • High-risk TB testing/screening (if patient meets criteria for high–risk)

40-64 Years Old

  • Hemoglobin and hematocrit (CBC)

  • Baseline EKG (once between 40-64)
  • Sigmoidoscopy (baseline at 45, then every 3-5 years after)
